InaccordancewithMassachusettsGeneralLawc.149,§27B,atrueandaccuraterecord

mustbekeptofallpersonsemployedonthepublicworksprojectforwhichtheenclosed

rateshavebeenprovided,APayrollFormhasbeenprintedonthereverseofthispageand

includesallthe

informationrequiredtobekeptbylaw.Everycontractororsubcontractoris

requiredtokeeptheserecordsandpreservethemforaperiodofthreeyearsfromthedate

ofcompletionofthecontract.

Inaddition,everycontractorandsubcontractorisrequiredtosubmit,onaweeklybasis,a

copyofhisorherweeklypayrollrecordstotheawardingauthority.Foreveryweekinwhich

anapprenticeisemployed,aphotocopyoftheapprentice'sidentificationcardmustbe

attachedtothepayrollreport.Oncecollected,theawardingauthorityisalsorequiredto

preservethosereportsforthreeyears.

Inaddition,eachsuchcontractor,subcontractor,orpublicbodyshallfurnishtothe

awardingauthoritydirectly,withinfifteendaysaftercompletionofitsportionofthework,

astatement,executedbythecontractor,subcontractororpublicbodywhosupervisesthe

paymentofwages,inthefollowingform:
